Ontario Health, in partnership with the Provincial Council for Maternal and Child Health (PCMCH), is seeking feedback on a draft quality standard on Transitions from Youth to Adult Health Care Services. Quality standards inform patients, clinicians and organizations about what high-quality health care looks like, based on the best available evidence.
About the Transitions from Youth to Adult Health Care Services Quality Standard:
This quality standard addresses care for young people aged 15 to 24 years who are transitioning from youth to adult health care services. It includes all pediatric and adult health care settings and providers relevant to the transition to adult health care services.
The scope of this quality standard includes all clinical populations, including groups with special health care needs such as young people with chronic and/or complex physical, intellectual, or developmental conditions and/or with mental health and addiction issues.
This quality standard will be open for feedback from November 8, 2021 to November 22, 2021.
