Client Rights and Responsibilities

You have the Right to:

  • Get information about your child’s care in a way you can understand
  • Be actively involved in your child’s care
  • Receive care that is safe, fair, and focused on what is important to you
  • Understand and agree to any treatment or care before it is provided
  • Have your child’s health information kept private and confidential
  • Share any concerns you have an get answers to your questions
  • Receive care that respects your language, culture, beliefs, and family structure

You have the Responsibility for:

  • Being respectful to others, including staff, clients, and visitors
  • Being an active partner in your child’s care; telling us what’s important to you
  • Arriving on time for your appointments
  • Letting us know if you or child are ill or if you’re unable to attend your appointment
  • Telling us if there are any changes to your child’s health or family’s information
  • Telling us if you have any concerns about your child’s care
