Transition to Adulthood

Muscular Dystrophy Canada (MDC) is excited to announce an engaging conference that will serve as an opportunity to connect, network and learn. Co-chaired by two adults living with spinal muscular atrophy, this one-day virtual conference will focus on making the transition from pediatric to adult care health care and community settings. The focus will be to primarily cover the transition to adulthood and adult experiences with healthcare systems and in the community. During the course of this 1-day conference, participants will:

  • Hear from thought leaders and peers regarding their experiences and recommendations for supporting youth transitioning to adulthood;
  • Learn about the neuromuscular transition experience via plenary talks, moderated panel discussions and audience participation;
  • Explore existing research, policy, and practices in adolescent and adult systems in Ontario, incorporating diverse perspectives;
  • Reflect on panelists’ experiences and receive tips on transitioning to adulthood for youth, parents, and clinicians;
  • Become equipped to make individualized informed choices with the information shared;
  • Learn about clinical and research updates in the areas of women’s health and multi-disciplinary wellness approaches;
  • Exchange ideas and make connections;
  • Commit to sharing and applying learnings to improve how we work together to support young people in the transition to adulthood and participate in live polling to capture viewpoints on experiences and preferences for different approaches.
Following the meeting, a report will be published that articulates the takeaways of the day.
